Get an A+ in health. Report your child's school immunizations

School-Aged Children

Ontario's Immunization of School Pupils Act (ISPA) requires parents of children attending elementary and secondary school provide their local health unit with proof of their child's immunization against the following designate diseases:

  • measles
  • mumps
  • rubella
  • diphtheria
  • tetanus
  • polio
  • meningococcal disease
  • whooping cough (pertussis)
  • chickenpox (only applies to kids born on or after 2010)

All the vaccines needed for school are free. Make an appointment with your health care provider if your child needs any of the above vaccines. It is the parent's responsibility to update their child's immunization record or provide a valid exemption form to Toronto Public Health.  Students without the required vaccines, will receive a letter from us, asking for updated immunization information. To avoid school suspension, please update your child's record with us every time your child receives a vaccination or booster. For more information visit: http://ow.ly/z3lw3048PTn vaccines1
